Minor Home ModificationsMinor home modifications can be applied to your home to support improved independence, safety and mobility. Minor home modifications can include the installation of grab rails in a shower, pool hoists, ramps (permanent and temporary), internal and external hand rails next to steps, installation of non-slip surfaces, and the installation and fitting of emergency alarms.

Equipment PrescriptionsAssistive technology can be equipment, aids or devices that can make your life easier, safer, or support you to achieve greater independence. Some equipment or items require an assessment to determine their suitability in line with your goal, needs and situation. Simply Engage OT can support you to find the most suitable equipment and conduct the necessary justification to help you with accessing it.

Treatment ReportsThe treatment cycle aims to improve the quality of care by supporting better coordination and communication between general practitioners (GPs), allied health providers and clients. At the end of your 12-month treatment cycle a report detailing your treatment and progress to date, along with future support recommendations will be submitted to your referring GP.

Daily Living Skills & Psychosocial Health TherapyThe goal of daily living therapy supports is to help you improve your independence and enable participation in daily and practical activities. This covers communication, mobility, movement, personal care, interpersonal interactions and community living. Occupational Therapy can help you develop your functional skills to improve your participation and independence.

What is Short-Term Restorative Care Support?The Short-Term Restorative Care (STRC) Programme is early intervention to reverse or slow ‘functional decline’ in older people. If you are having difficulty performing daily activities an occupational therapist can assist you with the necessary support and/or equipment to help you remain or stay in your own home for longer and to delay or avoid long-term care. Support is available for up to 8 weeks.

What is the Transition Care Program?The Transition Care Program (TCP) is designed to help older adults regain and maintain their independence following a hospital stay. It gives them the opportunity to engage in recovery support in the comfort of their home with the view to help them remain in home for longer and be safe whilst staying at home.
